How Can I Apply For Funding? Am I Eligible?
- Ask your doctor for a referral to the medical therapy specialist or psychologist.
- Ask your doctor for a referral to the CJ Riders Foundation for funding possibilities.
- For social needs ask your social worker for a referral to the CJ Riders Foundation.
- If you don't have a social worker you can still submit an application for funding possibilities.

Application Process
Get a Referral from Your Provider
Your doctor or social worker can refer you to the CJ Riders Foundation for funding options.
Complete and Send the Application
Fill out the form and answer each question to apply for funding.
We Review and Fund Your Support
If approved, we’ll decide the amount and pay your provider directly.
